About Hospice of Franklin County
Our hospice team of nurses, social workers, certified hospice aides, chaplains, volunteers, medical director and administrative staff all work together in the same office. Many of us have been providing hospice care in this area for over 20 years. The interdisciplinary team works with other community professionals to deliver all aspects of our hospice services. Although we spend most of our day visiting patients in their homes, our team meets at the beginning and end of each workday to support one another's practice with ideas, options, and suggestions that help lead to solutions to problems our patients are facing. Working more as a family ourselves, we are better able to support and give the best possible care to our patients and their families.
We belong to the community we serve. Our administrators and our Board of Directors live and work in Franklin County. We work with them face to face; they are accessible and responsive to the needs of our organization. Our managers understand that even small concerns can be very big to our patients, and they support our attention to these concerns in our practice.
We are committed to the core principles of hospice; to compassionately care for patients at end-of-life by managing and treating their pain and suffering; to support them by addressing their physical, emotional, spiritual, social, and bereavement needs as well as those of their families. We are committed to giving the very best care we have to offer to people in their homes, nursing homes, assisted living facilities or in the hospital.
Hospice of Franklin County was incorporated as a free-standing, non-profit, community based hospice in 2004. It was created specifically to serve community members of the greater Franklin County area and we have been priviledged to serve well over 1,500 individuals and their families in our community since first opening our doors. Hospice of Franklin County is licensed by the Department of Public Health and is a certified Medicare and Medicaid provider. Our services are also covered by most HMO's and private insurances.
Hospice of Franklin County is a member of National Hospice and Palliative Care Organization and the Hospice and Palliative Care Federation of Massachusetts.
