Hospice
of Franklin County includes a team of nurses, social workers, home
health aides, chaplains, volunteers, medical director and
administrative staff working together in the same office. This group,
known as an interdisciplinary team, works with other professionals to
deliver all aspects of hospice care. Although we spend most of our day
visiting patients in their homes, our team meets at the beginning and
end of each workday. The team supports one another and each other's
practice with ideas, options, and suggestions that lead to solutions to
the problems our patients are facing. This way, our team is able to
better support our patients. Working more as a family ourselves, we can
give the best care to our patients and their families.
We
belong to the community we serve. Our administrators and our board
members live and work in Franklin County. We work with them face to
face; they are accessible and responsive to the needs of our
organization. Our managers understand that even small concerns can be
very big to our patients, and they support our attention to these
concerns in our practice.
We
are committed to the core principles of hospice: to compassionately
care for the dying patient through this difficult time by managing and
treating their pain and suffering; to support them and their families
by addressing their physical, emotional, spiritual, social, and
bereavement needs. We are committed to giving the very best we have to
offer to people in their homes, nursing homes, or in the hospital.
